(a) To justify the additional days added to one's contract, please complete the following form at the end of each month.
(b) Submit reports by the 10th of each month, for the previous month's activities.
(c) When form is "submitted", it is automatically sent to the HISD Athletic Director.
NOTE: All activities listed on this form MUST OCCUR outside of regular school hours, on non-school day.
Employee CAN NOT receive additional pay, imbursement or compensation for activities listed on this form.
